Whose details go onto section 5 and 6 of CRA01 form?

On the CRA01 form, whose details go onto section 5 and 6 of the form? Mine or companies details? I am asking because this is a confirmation for business adress and on section 5 it requires that I put a tax number so I don't know whether to write my personal or business tax number.

Section 5 would be your details and section 6 the company confirming your address.
