Where to find my tax number and an IRP5 form?

Where can I get hold of this IRP5 form and how do I find out what my tax registration number is?
My company does not supply me with anything.

They have a duty to supply you with an IRP5 which is the summary of your payslips for the year. You can phone 0800 00 7277 to request your tax number.
