Where do you fill in the amount you paid out of your pocket for unpaid medical bills?

I have receipts of more than R5000 of bills of which medical did not want to settle. Where do I put this total on your questionnaire?

There will be a question asking you if you have any out of pocket medical expenses not covered by your medical aid.
