What penalties will I incur if I do not pay my donations tax within the three month deadline?

You would need to speak to SARS and ask them to reverse the penalties if they imposed them and provide details - assuming they do impose the penalty.

Not yet - it is merely an enquiry as this is the first tax year that I need to do this on behalf of my mom and we are going to be late with the payment!

It has not been SARS practice from what we can tell to penalise and if they do you can request to have this reversed if the reasons are valid.
