Please help regarding medical contributions?

My husband has an IRP5 from his employer and a income tax certificate from his medical aid. His work pays 50% of the medical aid and he pays 50%. But his 50% is deducted before he gets his wages. I am filling in a tax return for him and under Deductions on the IRP5 there are 3 amounts: codes 4003, 4005, 4474. I have filled that part in fine but the next page has Medical deductions from the taxpayer, there is a code 4040 to fill in, which I am not sure what to fill in. The amount on the medical aid form is not the same as the amount on the IRP5. Do I fill in the amount from the IRP5 or the amount on the medical aid certificate or nothing?

Is there an amount on his IRP5 for source code 3810? Does the amount next to source code 4005 match the medical aid certificate and does the amount for 4474 x 2 equal the total contribution?

There is an amount on the IRP5 in the income received section code 3810 (which is the same amount as deduction, code 4474) the amount of code 4474 x 2 does equal 4005 but the amount on the medical aid certificate is different, it is more than the amount of code 4005.

Do you know how much you pay each year for medical aid, which is the correct amount? If the medical aid goes through your employer then you do not need to complete the 4040 line. This is for medical expenditure not related to employment payments. You would only include the number of members in the top part of that section of the return.
