Medical aid source codes 2014?

My question is with regard to medical aid source codes for February 2014. What codes should I use if the employer contributes 50% and the employee contributes 50%?

The Fringe Benefit is source code 3810 which goes under income, the full amount that us actually spent and paid to the fund is 4005 under the Deductions side and the amount which equals the fringe benefit will go under 4474 on the deductions side.
