eFiling registration form's name.

I need to fill in the registration form and fax it to SARS together with my ID copy so that my SARS eFiling registration is completed. My problem is that I cannot find the registration form, what is the name of this registration form? When I received an sms from SARS to fill in this form, the name was not there so I am stuck, please help.

It will be on your SARS eFiling profile, go to USER and then Pending Registration and look there for the form.
