I still stay with my parents and have to go verify banking details at SARS. They sent me back to fill in a CRA01 form because the address is not in my name. My question is do I fill in the form or my parents fill it in?

Your parents complete it for you. You need to provide certified copies of their ID and the agreement showing they live in that address.
