Correct codes for medical expenses?

I need to know which code I need to use when adding an amount to medical cost incurred and paid for by myself.

You wouldn't add any codes, but would make use of the 4020 code on your ITR12 on the medical section where it asks for medical expenses not as a part of medical aid.
