Claiming out-of-pocket medical expenses when you don't belong to a medical aid

I have captured them under code 4020 but when I look at the notice of assessment, the amount to be refunded back to me is exactly the amount on my PAYE source code and it does not seem to take into consideration the out of pocket medical expenses I incurred... How does this work..?
